Before we come into a home, we meet with the patient and other family members who are critical to care. This first consultation will help us get to know your family, background, hobbies, and other needs. Our healthcare team will make note of any concerns you have and how you anticipate care to be administered.

Create a Care Plan
Our team will create a plan that best fits the patient’s lifestyle and needs. Some patients will need more hands-on care, while others may benefit from weekly chats or transportation to doctors’ appointments. Creating a care plan will ensure that no small need is missed and our caregivers are fully informed before they arrive.

Care Implementation
Once we have developed a plan you are happy with, we will begin serving your loved one and working around their home. Our caregivers will generally stick with the same patient for the long term, unless circumstances arise. This helps them to build trust and rapport with the patient to provide higher-quality care.

When a patient is stubborn and refuses care, it can be difficult to keep them safe. Our team will do our best to help your loved one, build trust, offer choices on how care is provided, and help boost their confidence. While the ultimate goal of caregiving is to limit strain on the patient, some may take a little convincing to let someone take over completely. We can start off with smaller tasks and limited appointments until the patient is more relaxed and comfortable with our team.

What If My Loved One’s Needs Change?
As time progresses, your loved one may need to change the services provided by our team. They may heal from a surgery or fall and need less help, or their condition may worsen and they need more one-on-one care. Comfort Home Care can meet with you annually to discuss how services are going and the condition of the patient. Our personalized plans allow us to have some flexibility in providing assistance and to change them as time goes on. If your needs change or another caregiver is going out of town, we can adjust provider times to help accommodate for them.
